RIGHTS
You have the right:
- to courteous, kind, competent care
- to collaborative care with your primary physician, or any other specialist
- to ask questions (questions are good)
- to an explanation of the recommendations made
- to obtain a second opinion
- to seek education and counseling elsewhere
- to know my credentials, my scope of practice, my training and my education
- to know if I feel I cannot help you
- to utilize medical and alternative and or adjunctive treatment options (just please let me know)
- to emergency treatment and crisis intervention 24 hours a day, however I can not be available to provide you with that; if you have an emergency when I am not available, please call 911, go to the local emergency department or call the crisis hotline (see list at the end)
- to a reasonable fee for my service
- to your own thoughts feelings and behaviors (though no verbal abuse will be tolerated)
- not to be judged or shamed if you do not accept or honor the recommendations made in these sessions
- to the confidentiality and privacy of the information you share
RESPONSIBILITIES
You are responsible for:
- being honest
- seeking additional information and additional help
- for not giving up
- notifying me if you feel what we are discussing and doing is not helping, so I can reevaluate and redirect your care
- being at least 18 years old, and providing me with your birth date
- keeping your bill current
